Resource Liaison
You’ve just received the results of your child’s assessment, and you’re now wondering what’s next. This may be an overwhelming time for you and your family as you try to navigate the system of referrals and service providers. You’ve been told to fill out some forms and connect with some agencies, but it all seems like a blur; you feel like you’ve been left in the dark and you now need some guidance. Our resource liaison nurse is here to facilitate this task by guiding you through the next steps, including funding applications and other paperwork. Together, we will review your assessment results and recommendations, and plan your child’s service needs accordingly. Our goal is to bridge the gap that exists between your clinical assessment and the community services; you shouldn’t have to walk that path alone.
